Each client has an experienced Registered Nurse as their Case Manager - regardless of package level, each Client can receive guidance and support.

CHT offer our Home Care clients priority access to our Residential Aged Care homes at either Hawthorn Village or Snug Village for your peace of mind should the need arise.
