The Aegis Ascot Transitional Care Program (TCP) is a unique and innovative pilot program, jointly funded by the State and Commonwealth Governments.It provides short term rehabilitative care for post hospital frail aged or those in the community requiring assistance to maintain independence in either their own home or in a comfortable home like environment.

The residential wings at SwanCare Kingia have been furnished to create a world of colour to aid in stimulation for residents. High attention to detail has been paid to ensure that furnishings encourage residents to engage with the world around them.

Located in Innaloo, Brightwater Birralee is a suburban home for a group of 30 permanent residents and 30 transition care clients, providing high quality care for people with different needs, including those with advanced dementia.Each house has an inviting and homely feel, with its own living room, kitchen, dining room and small library. Expansive gardens and gazebo areas surround each of the houses, providing plenty of spaces to relax outdoors. The home has won two Commonwealth Better Practice Awards from the Australian Aged Care Quality Agency. This prestigious national award recognises outstanding innovations in care. You'll find a large sensory garden which include quiet reflective areas for residents, family and friends. We provide frequent organised activities in the garden and encourage residents to use the garden to create a sense of wellbeing and freedom of movement. We also have a professional hairdresser who visits our home frequently - the cost of the service is added to your monthly bill.The 2015 Better Practice Award rewarded the home for our approach to supporting the care needs of older people of diverse sexual orientations. Through a multi-disciplinary program, our staff have been able to foster a culture within the home that is inclusive and accepting of lesbian, gay, bisexual, transgender and intersex (LGBTI) residents.
The 2017 Better Practice Award recognised the support and care taken to ensure care recipients’ final wishes being identified and respected. Transition Care Bull Creek is an 11-bed older adult mental health (OAMH) secure facility.
Residents have ensuite rooms and enjoy well equipped therapy rooms, lounge/dining room and well tended courtyard areas.The Transition Care Program is a Commonwealth-State initiative that provides short-term restorative care for older people after a hospital stay.
The aim is to help them regain independence and give them time to make a decision about their longer-term living options.
The average length of stay in transition care is seven weeks, clients may stay for up to 12 weeks if they need extra time.
