The Aegis Ascot Transitional Care Program (TCP) is a unique and innovative pilot program, jointly funded by the State and Commonwealth Governments.It provides short term rehabilitative care for post hospital frail aged or those in the community requiring assistance to maintain independence in either their own home or in a comfortable home like environment.
Juniper Hayloft is a welcoming, vibrant and beautifully designed residential aged care home where residents can enjoy modern amenities connected seamlessly to the natural surroundings.Spread across two storeys, the contemporary home has largesingle rooms with private ensuite, all with beautiful views of the surrounding bushland, open spaces or gardens. Abundant indoor and outdoor communal spaces enable residents to connect for dining, activities, or lounges or find a quiet nook to relax. Residents, their families and visitors are free to enjoy the playground andaccessible walking tracks.
A wellness centre is focussed on promoting wellbeing and independence and for those residents that choose, Hayloft runs an engagement and enablement program that promotes independence, wellbeing and purpose.

Tuohy is located in the beautiful Swan Valley region, one of Western Australia's oldest wine-producing regions and home to some lovely restaurants and cafes. Our bright and welcoming home offers large activity areas and lounges, all surrounded by beautiful gardens.
The home has won two Commonwealth Better Practice Awards from the Australian Aged Care Quality Agency. This prestigious national award recognises outstanding innovations in care. You'll find a large sensory garden which include quiet reflective areas for residents, family and friends. We provide frequent organised activities in the garden and encourage residents to use the garden to create a sense of wellbeing and freedom of movement. We also have a professional hairdresser who visits our home frequently - the cost of the service is added to your monthly bill.The 2015 Better Practice Award rewarded the home for our approach to supporting the care needs of older people of diverse sexual orientations. Through a multi-disciplinary program, our staff have been able to foster a culture within the home that is inclusive and accepting of lesbian, gay, bisexual, transgender and intersex (LGBTI) residents.
The 2017 Better Practice Award recognised the support and care taken to ensure care recipients’ final wishes being identified and respected. Partnering with care recipients, their representatives, care staff and community links, we developed the My Life, My Wishes booklet to document residents’ life stories, preferences and requirements.We are proud of our demonstrated leadership in providing inclusive aged care and welcome all people regardless of lifestyle choices, ethnicity, faith, sexual orientation or gender identity. Our current team work with residents to ensure their needs are met with inclusive and appropriate actions.
