Transition Care Bull Creek is an 11-bed older adult mental health (OAMH) secure facility. Residents have ensuite rooms and enjoy well equipped therapy rooms, lounge/dining room and well tended courtyard areas.The Transition Care Program is a Commonwealth-State initiative that provides short-term restorative care for older people after a hospital stay. The aim is to help them regain independence and give them time to make a decision about their longer-term living options. The average length of stay in transition care is seven weeks, clients may stay for up to 12 weeks if they need extra time.
