Overview
Each Sacred Heart Local client has an individual case manager to coordinate the support best suited to their situation and needs. Our staff are reliable, trustworthy, form strong connections and support clients to achieve their goals. Sacred Heart Local provides services under the Commonwealth Home Support Programme (CHSP), or Home Care Package (HCP) for people:
- 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people), or
- 50 years or older (45 years or older for Aboriginal Torres Strait Islander people) and on a low income, homeless, or at risk of being homeless.
