Health services

Our clinical care team can provide a range of services to help you manage your health conditions, support you after a hospital visit or surgery, or care at home to keep you out of hospital in the comfort of your own home. With your consent, we will reach out to your GP or current medical team for any information that may be required.

Nursing services

Our nursing team can treat wounds and assist with diabetes management. We can also help you care for your catheter and manage your medications.

Chronic disease services

If you live in South Eastern Melbourne, our Care Coordination Support Service can provide you with help to navigate your health condition, assistance to manage any changes, and support your ongoing care needs. This includes care coordination for long term conditions such as diabetes, cancer, and heart disease.
