As a registered Home Care Package provider, Trilogy Care offers two self-managed home care options: Self Managed and Self Managed PLUS.


Self Managed

Our Self Managed option is ideal for those who want complete control over their care services. This service includes:

  • A dedicated care partner, available for direct contact via phone
  • Your personalised care plan, with regular reviews based on your evolving care needs
  • Access to a team of clinical nurses to guide you on your care journey
  • Compliance checks for your care providers
  • Budget management
  • 15% of your Home Care Package, with no hidden fees or surcharges


Self Managed PLUS

Our Self Managed PLUS option is designed for those who would like some assistance with care coordination. This package includes:

  • A dedicated care partner, available for direct contact via phone
  • Your personalised care plan, with regular reviews based on your evolving care needs
  • Access to a team of clinical nurses to guide you on your care journey
  • Compliance checks for your care providers
  • Budget management
  • Care coordination support
  • 26% of your Home Care Package, with no hidden fees or surcharges


Transparent Pricing

Both our Self Managed and Self Managed PLUS options are offered at a flat percentage fee based on your approved Home Care Package budget. We don’t charge daily management fees, hidden costs, or surcharges! Our transparent pricing model ensures that you can maximise your budget for the care and support that matters most to you.
