Uniting AgeWell Tasmania North West Home Care support people in Tasmania’s north west to remain living at home, safely and independently, for as long as possible.
We provide Home Care Packages to seniors in Burnie, Central Coast, Devonport, Kentish, Latrobe and Waratah-Wynyard.
Home Care Packages support you with assistance such as:
- Personal and domestic care
- Clinical care
- Assistive technology
- Support with shopping
- Gardening
- Assistance attending a social activity
- Other community support.
The amount of support provided to you depends on the Home Care Package level you receive.
We also offer home based Transition Care Programs for the North West of Tasmania. If you are entering hospital for surgery and think you might need support when discharged please contact us or your doctor about being approved for a Transition Care Package (TCP) before your discharge.
